The legacy of a crowded ocean: indicators, status, and trends of anthropogenic pressures in the California Current ecosystem

As human population size and demand for seafood and other marine resources increase, understanding the influence of human activities in the ocean and on land becomes increasingly critical to the management and conservation of marine resources. In order to account for human influence on marine ecosystems while making management decisions, linkages between various anthropogenic pressures and ecosystem components need to be determined. Those linkages cannot be drawn until it is known how different pressures have been changing over time. This paper identifies indicators and develops time series for 22 anthropogenic pressures acting on the USA's portion of the California Current ecosystem. Time series suggest that seven pressures have decreased and two have increased over the short term, while five pressures were above and two pressures were below long-term means. Cumulative indices of anthropogenic pressures suggest a slight decrease in pressures in the 2000s compared to the preceding few decades. Dynamic factor analysis revealed four common trends that sufficiently explained the temporal variation found among all anthropogenic pressures. This reduced set of time series will be a useful tool to determine whether links exist between individual or multiple pressures and various ecosystem components.
Institutional Control Measures to Curtail the Epidemic Spread of Carbapenem-Resistant Klebsiella pneumoniae: A 4-Year Perspective

Objective.
To describe the implementation of an institution-wide, multiple-step intervention to curtail the epidemic spread of carbapenem-resistant Klebsiella pneumoniae (CRKP).
Design.Consecutive intervention analyses.
Patients and Setting.All patients admitted to a 775-bed tertiary care medical center in Jerusalem, Israel, from 2006 through 2010.
Interventions.The effects of 4 interventions were assessed: (1) a policy of isolation for patients colonized or infected with CRKP in single rooms, which was started in March 2006; (2) cohorting of CRKP patients with dedicated nursing staff and screening of patients neighboring a patient newly identified as a carrier of CRKP, which was started in March 2007; (3) weekly active surveillance of intensive care unit patients, which was started during August 2008; and (4) selective surveillance of patients admitted to the emergency department, which was started in March 2009. Interrupted regression analysis and change-point analysis were used to assess the effect of each intervention on the CRKP epidemic.
Results.Patient isolation alone failed to control the spread of CRKP, with incidence increasing to a peak of 30 new cases per 1,000 hospital beds per month. Institution of patient cohorting led to a steep decline in the incidence of CRKP acquisition (P< .001). Introduction of active surveillance interventions was followed by a decrease in the incidence of CRKP-positive clinical cultures but an increase in the incidence of CRKP-positive screening cultures. The mean prevalence of CRKP positivity for the period after cohorting began showed a statistically significant change from the mean prevalence in the preceding period (P< .001).
Conclusions.The cohorting of patients with dedicated staff, combined with implementation of focused active surveillance, effectively terminated the epidemic spread of CRKP. Cohorting reduced cross-infection within the hospital, and active surveillance allowed for earlier detection of carrier status. Both interventions should be considered in attempts to contain a hospital epidemic.

Objective.
To determine risk factors and outcomes associated with ciprofloxacin resistance in clinical bacterial isolates from intensive care unit (ICU) patients.
Design.Prospective cohort study.
Setting.Twenty-bed medical-surgical ICU in a Canadian tertiary care teaching hospital.
Patients.All patients admitted to the ICU with a stay of at least 72 hours between January 1 and December 31, 2003.
Methods.Prospective surveillance to determine patient comorbidities, use of medical devices, nosocomial infections, use of antimicrobials, and outcomes. Characteristics of patients with a ciprofloxacin-resistant gram-negative bacterial organism were compared with characteristics of patients without these pathogens.
Results.Ciprofloxacin-resistant organisms were recovered from 20 (6%) of 338 ICU patients, representing 38 (21%) of 178 nonduplicate isolates of gram-negative bacilli. Forty-nine percent of Pseudomonas aeruginosa isolates and 29% of Escherichia coli isolates were resistant to ciprofloxacin. In a multivariate analysis, independent risk factors associated with the recovery of a ciprofloxacin-resistant organism included duration of prior treatment with ciprofloxacin (relative risk [RR], 1.15 per day [95% confidence interval {CI}, 1.08-1.23]; P < .001), duration of prior treatment with levofloxacin (RR, 1.39 per day [95% CI, 1.01-1.91]; P = .04), and length of hospital stay prior to ICU admission (RR, 1.02 per day [95% CI, 1.01-1.03]; P = .005). Neither ICU mortality (15% of patients with a ciprofloxacin-resistant isolate vs 23% of patients with a ciprofloxacin-susceptible isolate; P = .58 ) nor in-hospital mortality (30% vs 34%; P = .81 ) were statistically significantly associated with ciprofloxacin resistance.
Conclusions.ICU patients are at risk of developing infections due to ciprofloxacin-resistant organisms. Variables associated with ciprofloxacin resistance include prior use of fluoroquinolones and duration of hospitalization prior to ICU admission. Recognition of these risk factors may influence antibiotic treatment decisions.
